CVS Health Survey at CVSHealthSurvey.com (17-digit Survey ID guide)

Last reviewed 12 September 2026Independent guide, not run by CVS

The CVS Health Survey is the short feedback survey printed on CVS Pharmacy receipts. You open www.cvshealthsurvey.com, type the 17-digit Survey ID from your receipt, and rate the visit. No login, no ExtraCare card, and the receipt itself says it takes about a minute.

One thing to know first. The official survey page says nothing about a prize. The $1,000 gift card promised on look-alike sites isn’t on the survey CVS runs in 2026, and I couldn’t find official rules for it anywhere.

What is the CVS Health Survey?

It’s a customer satisfaction survey, the same kind of receipt survey Walgreens and Kroger run, and CVS uses it to score each store on the visit you just made. The survey is hosted by Medallia, which is why the address changes to survey.medallia.com when you open it.

The “Health” in the name comes from the parent company, CVS Health, and it confuses people. CVS Health also runs an employee engagement survey and a public health poll. Neither uses a receipt. If you’re holding a receipt with a Survey ID on it, this page is the one you want.

The chain is simple. You buy something, the receipt may print a Survey ID, the ID opens the survey, and your answers land on that store’s scorecard.

Which website is the official one?

The only address CVS prints is www.cvshealthsurvey.com. On 12 September 2026 it forwarded to survey.medallia.com/?cvs-drivers, a plain page with the CVS Pharmacy logo, a language box, one text field and a red Next button.

The footer links go to Medallia’s support page and to CVS’s own privacy policy on cvs.com. That’s how you can tell the redirect is genuine.

There’s no account and no password. The Survey ID is the only thing you ever enter. If a page asks for a card number, a shipping fee or a password before showing the 17-digit box, it isn’t CVS. Close it and type the address from the receipt.

Where is the CVS survey ID on the receipt?

It’s printed in the middle of the receipt, under the item list and above the subtotal, in bigger type than everything else, with the label Survey ID #. It’s 17 digits grouped 4-4-4-3-2, like 7270 5923 8098 502 75.

CVS Pharmacy receipt with the 17-digit Survey ID printed above the total and the survey address at the bottom
Survey ID in the middle, survey address at the bottom. The number under the barcode is the transaction number, not the Survey ID.

One trap. A second 17-digit number sits under the barcode, near the “Returns with receipt” line. That’s the transaction number for refunds, and the survey page rejects it. Use only the number under the words Survey ID #.

You don’t need the store, register or transaction numbers from the top line either. Every CVS store uses the same site and the same 17-digit format.

Why some CVS receipts have no Survey ID

Not every receipt gets one, and most guides skip this. CVS store employees say purchases with an ExtraCare card are picked at random for a survey, while purchases without a card print one every time. I can’t confirm the exact rule, but shoppers report the same thing.

If there’s no Survey ID, you can’t force one. Call 1-800-SHOP-CVS (1-800-746-7287) instead, which is what a CVS pharmacist-in-charge recommends for feedback without a receipt. Lines are open weekdays 8:30 AM to 7 PM ET and weekends 10 AM to 6:30 PM ET.

What you need before you start

  • A CVS receipt that shows a “Survey ID #” line. No line, no survey.
  • The 17 digits, readable. Faded thermal print is the number-one cause of “invalid ID”.
  • Any browser on a phone or computer. There’s no app.
  • Two or three minutes.
  • Nothing else. No email, no password, no card details.

Type or paste the number printed under “Survey ID #” on your receipt.

This only counts digits in your browser. It doesn’t send the number anywhere. To take the survey, use the official page.

How to do the CVS survey step by step

One screen for the ID, then a short run of rating questions. Here’s the order, with screenshots of the real pages.

  1. Find the Survey ID on your CVS receipt

Look for the line “Survey ID #” in the middle of the receipt, in larger type between the item list and the total. It’s 17 digits grouped 4-4-4-3-2. If that line isn’t there, this receipt wasn’t selected.

  2. Open www.cvshealthsurvey.com

Type the address from the bottom of the receipt into your browser. It forwards to survey.medallia.com, which is the real survey page, so the Medallia name in the address bar is expected.

  3. Pick English or Español

The language box sits at the top right. Choose Español if you want the questions in Spanish; that’s what the “Hablamos español” line on the receipt means.

  4. Type the 17 digits and press Next

Enter the Survey ID exactly as printed; spaces don’t matter. If the page says the ID is invalid, compare each digit with the receipt. A faded 3 that looks like an 8 is the usual problem.

  5. Rate your visit on the 0 to 10 scales

You’ll rate things like finding items and checkout time, then answer yes-or-no questions about cleanliness and stock. Use the N/A circle when a question doesn’t apply.

  6. Add a comment, then submit

When the comment box appears, name the employee, the department and what happened. Press Next until the thank-you screen. There’s nothing to redeem; the survey ends there.

What you get at the end

A thank-you screen. No code to write on the receipt, no coupon. If you were expecting a validation code, that’s a different chain’s survey.

What questions does the CVS survey ask?

Short ones. The screenshot above shows the pattern: 0 to 10 scales from “Not At All Satisfied” to “Extremely Satisfied” with an N/A option, then yes-or-no questions. A recent front-store survey asked about finding items, checkout time, whether the store was clean and whether everything was in stock.

How the scores count matters more than the questions. CVS employees describe it as a Net Promoter style system where 9 and 10 count as positive, 7 and 8 count as zero, and 6 or below counts against the store. I’m taking their word for the exact weights, but they all say the same.

So here’s my advice. If one employee helped you, give the 10 and put their name in the comment box. That’s what the survey is good for.

If your complaint is about prices, hours or an insurance rejection, store staff can’t fix any of it, and a 4 only hurts them. Call the 800 number instead, where the complaint reaches corporate.

Is the CVS survey legit, and is there really a $1,000 gift card?

The survey is legitimate. Medallia runs surveys for many big retailers, the page links to CVS’s privacy policy, and it asks for nothing but the receipt number.

The $1,000 gift card is another matter, and here I’ll disagree with most pages on the subject. In September 2026 the official survey page mentions no sweepstakes, the receipt mentions no sweepstakes, and no official rules are published anywhere I could find.

Graphic of a $1,000 CVS gift card of the kind used by unofficial survey sites
The prize graphic unofficial sites use. Nothing like it appears on the survey CVS runs.

There was once something behind it. Former employees have said an earlier survey vendor did run a $1,000 sweepstakes, and one store had a customer win it around 2017. A pharmacist-in-charge said staff were later told to stop mentioning it because CVS no longer offered it.

My honest read: treat the survey as feedback only. If a sweepstakes screen ever appears after you submit, that’s a bonus, and it would come from the Medallia page, never from an email.

CVS survey scam emails and texts

There is a real CVS survey scam, and it has nothing to do with the receipt survey. CVS’s security page on cvs.com warns about emails titled “Congratulations on your CVS Survey Reward” that lead to sites unrelated to CVS, and says plainly that CVS Health doesn’t contact customers this way.

Truth in Advertising documented a version on 6 February 2025: a “CVS Rewards Team” message offering a free gift worth up to $100 for a short survey, followed by a robot vacuum or smartwatch that only needs you to pay shipping. Nobody gets a vacuum.

How to tell the real survey from the scam in five seconds

The real one starts with a receipt in your hand and a 17-digit box. The scam starts in your inbox, promises a gift, uses a countdown timer, and asks for a card number. Real ExtraCare mail comes from extracare@your.cvs.com. Anything else, delete it.

If you’ve already typed card details into one of these pages, call your bank first, then report the message at reportfraud.ftc.gov.

Medallia’s Survey Support page, linked in the survey footer, is a general help centre and won’t reset an ID for you. For anything about the store itself, the 800 number is faster.

Does CVS actually read the feedback?

Yes, and more concretely than the usual corporate line. Results feed a per-store dashboard that staff see through an internal tool, and store managers are measured on it monthly. CVS’s Active Listening page says a team reads and acts on survey comments, and credits customer feedback for retail training.

The flip side, which employees are candid about, is that the number grades stores; it doesn’t fix policy. A complaint about smaller shelf tags (a real example a worker gave) still counts against the store where you typed it.

Positive comments do get seen. One tech mentioned a glowing review being removed only because the system worked out the reviewer was an employee’s relative, which tells you somebody is reading.

CVS employee survey vs the customer survey

If you searched for the “CVS employee survey” or “engagement survey”, you want a different page. The CVS Health Colleague Voices Engagement Survey is the internal survey for staff, with results at voices.cvshealth.com behind a company login. Customers can’t take it.

Two more things share the name. The CVS Health–Harris Poll National Health Project is a monthly public-opinion poll on cvshealth.com, and the CVS Advisor Panel is an online research panel for ExtraCare members that pays in ExtraBucks. Its own site says it isn’t taking new members right now.

And a naming quirk: Longs Drugs stores in Hawaii are CVS stores under an older name, so a Longs receipt survey is the same survey with the same 17-digit ID.

Other ways to give CVS feedback

When there’s no Survey ID or the issue is bigger than one visit, you have three routes. Phone, 1-800-SHOP-CVS. The contact form under Help on cvs.com, which promises a reply within two business days. And for prescriptions, the separate Pharmacy Customer Relations form on cvs.com.

If you do these surveys often, the Kroger feedback survey and the Lowe’s survey still run published sweepstakes with real rules, which is an interesting contrast with CVS. Fast-food ones like TellTheBell and MyBKExperience give a validation code for a free item instead.

CVS Health Survey FAQs

The receipt and the survey page don’t print an expiry, so I can’t give you a verified number. Third-party sites say 7, 30 and 60 days, which tells you they’re guessing. Do it the same day or the next.

No. The first screen asks for a 17-digit Survey ID and won’t move on without one. If you lost the receipt, call 1-800-SHOP-CVS (1-800-746-7287) or use the contact form on cvs.com.

No login, no password, no ExtraCare card. The site never asks you to register. The Survey ID is the only thing you enter.

One receipt, one survey. Once a Survey ID has been submitted it’s finished. A new purchase gives you a new receipt and, if it prints, a new ID.

No. The Medallia page is a normal web page and works in Safari or Chrome on a phone, which is where most people finish it.

Only one is the Survey ID, the one under the words “Survey ID #” above the total. The other string, under the barcode near the returns line, is the transaction number, and the survey page rejects it.

CVS does email some ExtraCare members after a visit, and the Advisor Panel emails its members. But any email saying you’ve won a CVS survey reward or a free gift is phishing. Real ExtraCare mail comes from extracare@your.cvs.com.

The score goes to the store’s dashboard, which staff check through an internal tool. The comments are read by the store manager and, per CVS’s Active Listening page, a corporate team. Naming the employee who helped you is the most useful thing you can type.
